Health and Medical Forms
All students must have a student health form on file with the school; please download the form below and return it to your child's teacher during Open House on August 1st. If your child needs medication administered at school, please download the form below and have your physician complete it. On the first day of school, please place the form and medication in a Ziploc bag clearly labeled with your child's first/last name. Please bring these items into the school and give them to the school nurse. If you cannot, inform your child's teacher that these items are in your child's bookbag, and we will make sure the nurse receives them.
